Here are the steps to follow for the replacement of a lost/damaged passport at the Philippine Embassy in Kuwait:
1. Appointment Confirmation (To schedule a passport appointment, visit https://passport.gov.ph)
2. Personal appearance is required on the scheduled date and time of your confirmed appointment.
3. Complete the application form as instructed.
4. If available, provide the most recent Philippine passport – original and one (1) photocopy of the data page displaying your photo and full name.
5. Proof of Philippine Citizenship – original and (1) photocopy of one of the following, as applicable:
-
- Civil ID and a Philippine Government Issued ID. (See the List of Acceptable IDs here)
- Valid permanent/temporary residence card or work permit (if currently being applied for or renewed, please provide proof of your pending application for the license).
- Dual citizenship documents issued by the Philippine Bureau of Immigration or Philippine Embassy/Consulate General (Identification Certificate, Oath of Allegiance, and Order of Approval).
- Most recent tourist visa.
- Affidavit of Loss (if the passport is lost) or Affidavit of Mutilation (if the passport is damaged).
- An English-language police report (for lost passports).
- Birth Certificate (BC) in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) – original and (1) photocopy.
- If the entries on the birth certificate are unclear or unreadable, a transcribed Birth Certificate from the Local Civil Registry is required.
- If born abroad, the Report of Birth is duly authenticated by the Philippine Statistics Authority (PSA) or registered by the Philippine Embassy in Kuwait.
- For married women, a Marriage Certificate (MC) in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) – original and one (1) photocopy.
- If the entries on the marriage certificate are unclear or unreadable, a transcribed Marriage Certificate from the Local Civil Registry is required.
- If married abroad, the Report of Marriage is duly authenticated by PSA or registered by the Philippine Embassy in Kuwait within one year of applying for the child’s passport.
6. Payment of fees: KD 48.750 for replacing a lost ePassport, KD 19.500 for replacing a damaged ePassport. And one (1) photocopy of the data page displaying your photo and full name.
